Direct handoff
Use the approved GLP intake path for eligibility and clinical information. Concordia does not use ordinary Shopify forms to collect diagnoses, lab values, detailed symptoms, medication history, or other sensitive health information outside the approved intake pathway.
Completing an assessment does not guarantee eligibility, approval, treatment, prescription, medication access, shipment timing, weight loss, appetite change, health improvement, or results.
