How Concordia handles website, intake-routing, support, provider, pharmacy, payment, and communication information.

Effective date: July 9, 2026. Entity: Concordia Men’s Health, LLC. Website: concordiamenshealth.com.

Concordia Men's Health operates a website and related non-clinical coordination services that help patients access independently licensed telehealth providers and licensed U.S. pharmacy partners where clinically appropriate. This policy explains the categories of information that may be collected, used, shared, retained, and protected when you use the website, assessment route, patient portal links, support channels, checkout, provider pathway, pharmacy coordination process, or communications.

Information you provide

You may provide contact, account, eligibility, routing, payment, shipping, identity-verification, consent, and support information. Through the approved intake route, you may also provide health-history information, medications, allergies, goals, measurements, symptoms, photographs, laboratory information, or other information needed by a licensed provider to evaluate whether care is appropriate.

Information collected automatically

The website and service providers may collect technical information such as device type, browser type, operating system, IP address, pages visited, links clicked, timestamps, referral source, cookie identifiers, analytics events, approximate location derived from IP address, and similar usage information. These tools may support security, authentication, analytics, site operations, attribution, advertising measurement where permitted, and consent management.

Information from providers, pharmacies, platforms, and service providers

Concordia may receive limited information from independent medical providers, pharmacies, intake-platform vendors, payment processors, identity-verification vendors, shipping or fulfillment partners, analytics providers, communications tools, customer-support vendors, and security vendors when they assist in operating the website, assessment route, account, payment, clinical routing, or pharmacy coordination process.

How information is used

Information may be used to operate and improve the website, route assessment information to the appropriate intake path, support account creation, verify identity where required, coordinate provider review, coordinate pharmacy fulfillment if prescribed, process authorizations or payments, communicate transactional and care-related updates, provide support, detect fraud or abuse, analyze site performance, honor consent choices, comply with law, and enforce applicable terms.

Protected Health Information and HIPAA

When medical services are provided through an independent provider group or pharmacy partner, medical information created or received for diagnosis, treatment, prescription, or pharmacy fulfillment may be Protected Health Information, also called PHI. That information is governed by HIPAA, applicable state privacy laws, and the Notice of Privacy Practices made available by the applicable provider group or pharmacy. Concordia's administrative role does not replace the provider or pharmacy's clinical and privacy responsibilities.

Provider and pharmacy separation

Concordia provides education, routing, administrative support, technology coordination, and non-clinical care coordination. Concordia does not practice medicine, diagnose, prescribe, compound, dispense, or fulfill prescriptions. Clinical decisions are made by independent licensed providers, including DrTelx where available. Prescription fulfillment is handled by licensed U.S. pharmacies when a valid prescription exists.

How information may be shared

Information may be shared with independent licensed providers who review intake information and decide whether care is appropriate, licensed pharmacies that dispense prescribed medication, platform and service providers that operate the assessment, payment, account, communications, analytics, hosting, fraud-prevention, support, shipping, or security functions, and legal or safety recipients where required by law, court order, valid government request, safety concern, business transfer, or consent.

Service providers and vendors

Vendors may process information for hosting, security, analytics, communications, payment authorization, identity verification, customer support, appointment or portal workflows, shipping, fraud prevention, and similar operational purposes. Vendors are expected to use information only as needed to perform their services and comply with applicable obligations.

Cookies, analytics, and advertising choices

The website may use cookies, pixels, and similar technologies to keep the site working, measure performance, understand how users arrive, and support marketing measurement where permitted. You can manage cookies through your browser settings or available cookie controls. Some privacy laws may treat certain advertising or analytics disclosures as a sale or sharing of personal information, and eligible residents may exercise opt-out rights where required by law.

Communications and SMS

Concordia, the provider group, pharmacies, and service providers may send account, assessment, appointment, checkout, refill, support, transactional, or care-related communications by email, in-platform message, phone, automated call, prerecorded or artificial voice message, and text message where permitted and where consent has been provided. Standard message and data rates may apply. You may opt out of non-clinical marketing messages without affecting eligibility for care.

Payment information

Payment information may be collected and processed by payment processors and platform vendors. Concordia may receive transaction status, authorization, billing, refund, subscription, or fraud-prevention information needed to operate checkout and support processes, but full payment-card details are handled by the payment processor according to its own security obligations.

Security and retention

Reasonable administrative, technical, and physical safeguards should be used to protect personal information, including access controls, encryption in transit where appropriate, monitoring, vendor controls, and security review. No electronic system is perfectly secure. Information is retained as long as needed to provide services, maintain records, comply with law, resolve disputes, enforce terms, and support provider or pharmacy record-retention duties.

Your choices and privacy rights

You may request access, correction, deletion, portability, restriction, opt-out, or other rights where available under applicable law. State privacy laws, including California and other U.S. state privacy laws, may provide rights to know, access, correct, delete, limit, or opt out of certain processing. Medical record requests should be routed through the provider group or secure clinical pathway that maintains the medical record.

Marketing opt out

You may opt out of marketing emails by using an unsubscribe link where provided or by contacting support. Transactional, clinical, pharmacy, safety, account, billing, or legal messages may still be sent where permitted or required.

Children's privacy

Concordia services are intended for adults who are at least 18 years old. The website and services are not directed to children, and Concordia does not knowingly collect personal information from children under 13.

Do Not Track

Some browsers transmit Do Not Track signals. Because there is no uniform industry standard for responding to those signals, the website may not respond to them separately from available cookie controls or legally required opt-out mechanisms.

Business transfers

Information may be transferred or disclosed in connection with a merger, acquisition, financing, reorganization, sale of assets, bankruptcy, or similar business transaction, subject to applicable law and required protections for health information.

Changes to this policy

This policy may be updated as the website, provider pathway, pharmacy network, privacy law, or platform operations change. Continued use after an update means you accept the updated policy where permitted by law.

Contact

Privacy requests and general questions may be sent to support@concordiamenshealth.com unless a provider, pharmacy, portal, or applicable notice gives you a different contact path for clinical records or PHI requests.
